2025-2026 Eligibility, Consent to Participate, and Release Form
To be completed by parents/guardians to permit students to participate in high school athletics.

- Each school must complete this form for every team.
- Note: The linked document is a view-only template and cannot be edited directly.
- Two ways to complete the form:
- Download the file to Excel, or
- Select “Make a Copy” to use it in Google Sheets.
- Completed copies should be:
- Sent to your Conference Administrator, and
- Retained on file at the school.
- Do not submit this form to the NCHSAA.
