Moe’s Hometown Hero Nomination Form

The Moe’s Hometown Hero program seeks to offer a platform through which school administrators and personnel, parents, students, and school community members can nominate deserving individuals for special recognition on a monthly basis during the school year.

Currently, this new platform is available to a targeted group of NCHSAA member high schools located within a 12-mile distance from a Triangle Moe’s Southwest grill location.

Nominations are due by the 23rd of each month, August through December, in the 2018-19 school year. Submitted nominations will be reviewed by an NCHSAA selection committee who will then select up to three (3) individuals each month for recognition as A Moe’s Hometown Hero.

Selection credence is given to the following:

  • Participation and excellence in athletics and academics (students)
  • Community service and sound citizenship (all)
  • Making a difference in the lives of others (all)
  • Other qualities setting the individual apart from peers (all)

Monthly recipients will each receive a commemorative award and a $25.00 Moe’s Southwest Grill gift card to use at the local Moe’s location. The NCHSAA will facilitate a personal delivery of the award/gift card to each recipient by a member of the NCHSAA staff and a local Moe’s franchisee.

Nominations should be submitted each month by school administrators and/or personnel, parents, students, and community members through this online form.
