** The 8-quarter pre-participation form must be completed on and faxed to 919-962-7812 by 3:00 P.M. on the Friday of the varsity contest.
** The junior varsity player(s) MUST be examined by a MD/DO/Licensed Atletic Trainer FOLLOWING play in the junior varsity game on Thursday.
** An approval or denial form will be faxed to the school's athletic director acknowledging the reciept and approval or denial of the pre-participation form.
** The 8-quarter follow-up form must be completed and faxed to 919-962-7812 by 4:00 P.M. the Monday following the varsity contest.
