Official Coach/Player Ejection-DQ Report


Instructions: This form is to be completed by the Athletic Director and Official reporting the ejection OR disqualification of a player or coach. The form must electronically submitted, or faxed to the NCHSAA within 24 hours or 1 working day following the completion of the game. In accordance with NCHSAA HANDBOOK policy, a fine of FIFTY DOLLARS ($50.00) will be imposed for failure to submit this report to the NCHSAA by the deadline. Reports must be faxed or submitted to:
